I hereby grant Camp Shemesh permission to use my child(ren) in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Camp Shemesh and will not be returned.I hereby irrevocably authorize the Camp Shemesh to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose.*

In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. I waive any right to royalties or other compensation arising or related to the use of the photo.I hereby hold harmless, release, and forever discharge the Camp Shemesh from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.*

I, as the parent or legal guardian of the Participant, have carefully reviewed and hereby sign this form voluntarily. I acknowledge the legal implications associated with signing this document, which include the following: (a) releasing Camp Shemesh of Boca Raton from any liability on behalf of both myself and the Participant, (b) waiving our right to pursue legal action against Camp Shemesh, and (c) assuming all risks associated with the Participant's involvement in any activities or events.*

I grant permission for the Participant to engage in all activities. Furthermore, I understand that I am personally responsible for the Participant's actions and obligations as outlined in this form, including any damages to property or items. I willingly agree to abide by the terms and expectations set forth by Camp Shemesh.  I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT. *
